User account names

Issue description

Currently there is a couple of unclear issues regarding user accounts.

Developer comments

When editing a user account in the form, the name can currently be changed, but how does this relate to the record in the users table.

This jar of problems surfaced with [14031, this issue]. This bug caused several topic of type user account to be present in the system, but they were not related to a record in the users table.
