Visual focus mark is often only partially visible

Issue description

Whenever the focused element is positioned without 0 margin in either direction, the visual focus mark ist cut off and ugly. Think of a different way to mark the focus or position focusable elements always with sufficient margin.

Developer comments

Outline style %auto% would solve many of these problems.
